A double-blind, split-face RCT put a ceramide-and-niacinamide moisturizer on one side of the face and a plain hydrophilic cream on the other, with BOTH sides using the same adapalene 0.1% + benzoyl peroxide 5% regimen — the niacinamide/ceramide side cleared significantly more acne lesions by week 8. That's an efficacy synergy, not the assumed tolerability one: the same trial found no significant difference in irritation measures (transepidermal water loss, hemoglobin index, hydration, skin pH) between the two moisturizers, so the popular claim that niacinamide specifically eases adapalene irritation isn't actually what this study shows.
